Dear students who experienced technical problems in registering ADAPASS system;
The attached form has been prepared in order to enable you to register in the vaccine information system. You are expected to fill in the attached form and email it to med.studentaffairs@emu.edu.tr as soon as possible (latest by October 15, Friday 2pm).
In addition, a photo of your vaccination card with your vaccination information should be attached to the email.
